By checking this box you submit that you have read and agree to the Athlete Waiver Agreement and will be allowed to participate in the Sports Skills Development Clinic at William Rainey Harper College and specifically consent to your participation. As a participant in athletics, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume the full risk of any injuries, damages or loss which I may sustain as a result of participating in any and all activities with or associated with the Sports Skills Development Clinic whether undertaken as a group or individual. I agree to waive and relinquish all claims I may have as a result of the participation against William Rainey Harper College, its Board of Trustee, officers, representatives, agents, servants, and employees. I do hereby release and discharge Harper College, its Board of Trustee, officers, representatives, agents, servants, and employees from any and all claims resulting from injuries, damage or loss which I may have or which may accrue to me on account of participation in the Sports Skills Development Clinic. I further agree to indemnify and hold harmless and defend William Rainey Harper College, its Board of Trustee, officers, representatives, agents, servants, and employees from any and all claims resulting from injuries, damages and losses sustained by me and arising out of, connected with, or in any way associated with the activities of the Sports Skills Development Clinic. In the case of emergency, I authorize William Rainey Harper College officials to secure from any licensed hospital, physician and/or medical personnel any treatment deemed necessary for my immediate care, and I agree that I will be responsible for payment of any and all medical services rendered.
